I'm not sure how practical the theory test is, but I'd support a breath test for any driver with prior drink–driving convictions. Making it selective would add to the stigma and also avoid unnecessary cost for people who never drink and drive.
A reaction time test is an interesting idea. We struggled for years to get my father to give up his car, so I'm sympathetic to the idea. I guess it might also prevent some kinds of drug–driving.
