The article indicates that approvals for using these methods and facilities had been applied for but not cleared when they were used anyway. I suspect this was mostly just not waiting for paperwork stuff, which is often an issue in aerospace work. In other words an incomplete checklist. I also suspect that this happened many levels below Elon and he personally knew nothing about it. Keep in mind SpaceX has several layers of management whose sole purpose is to keep Elon away from the daily workings of the company, which is down where this happened. Minor screw up, no damage done, fine paid and probably the approvals arrived the next day anyway.
