yeah it will be brutal. The reality of say an Richter 8+ in LA area or SF means that IF the hospitals survive and that is a huge IF there will be no help coming. Whoever is on for fire, EMS, police etc will be on for the duration assuming they survived it as well and be saturated with calls.
This will be exacerbated in wealthy communities where nurses, doctors, fire, police EMS and other first responders cannot afford to live in the communities they serve.
In addition Hospitals and the like are predicated on running water, electricity, and a huge just in time supply chain.
Ha, when I lived in the east bay many moons ago they looked at all the hospitals, firestations, and other critical infrastructure on the Hayward fault - it was a LOT.

I think that is the plan almost everywhere.
