This study aims to identify patient-related factors contributing to polydoctoring in older adults in Japan. Polydoctoring was significantly associated with eye diseases, osteoporosis, prostate diseases, and osteoarthritis. No significant correlation was found with educational status or financial hardship.
Abstract
Background
Multimorbidity increases with age, leading to various adverse outcomes, including higher mortality, care dependency, hospitalizations, and healthcare costs. Polydoctoring, managing a patient with multimorbidity by multiple healthcare providers, can be a risk of fragmented care and increased healthcare expenditures. This study aims to identify patient-related factors contributing to polydoctoring in older adults with multimorbidity.
Methods
This study is a cross-sectional study using baseline data from the Kawasaki Aging and Wellbeing Project. Participants were residents of Kawasaki City aged 85–89 years, without disability in basic activities of daily living, and being able to visit study site. The regularly visited facilities (RVF) index was employed to quantify polydoctoring. Polydoctoring was defined as having two or more RVFs. Poisson regression analysis was conducted to assess the association between polydoctoring and patient demographics, including types of chronic conditions and socioeconomic factors.
Results
A total of, 968 participants with multimorbidity were analyzed. Increased RVF was significantly associated with eye diseases (rate ratio [RR] 1.27, 95% confidence interval [CI] 1.12–1.44), osteoporosis (RR 1.22, 95% CI 1.08–1.38), prostate diseases (RR 1.22, 95% CI 1.07–1.40), and osteoarthritis (RR 1.16, 95% CI 1.05–1.27). No significant correlation was found with educational status or financial hardship.
Conclusion
The study indicated that certain chronic conditions are linked to increased polydoctoring among multimorbid older adults in Japan. However, most of those conditions are considered to be within a scope of family medicine/general practice. Training general practitioners to manage these conditions could reduce healthcare costs and the treatment burden, indicating a direction for future healthcare policy and medical education.

TITLE: Are narrative story stem methods valid in “non-Western” contexts? A systematic review

International Journal of Behavioral Development, Ahead of Print. Narrative story stem techniques (NSSTs) offer insight into attachment and other representational aspects of preschool to young school aged children’s inner lives. While the method moved into the academic and clinical mainstream some 35 years ago, their applicability to “non-Western” contexts remains little understood. This synthesis comprises 31 NSST studies of samples from parts of Africa, East Asia, Latin America, and the Middle East, and from US and UK ethnocultural minoritized backgrounds. In the reviewed studies, three specific NSSTs dominated, story stems were used most to evaluate attachment, and some were clinically focused. However, there was also a strong cultural focus and over half of samples were socioeconomically disadvantaged. Studies revealed both universal and culturally specific features of NSSTs. Attachment distributions were as expected, given the high clinical risk in pooled samples (49% secure, 19% avoidant, 12% ambivalent, 20% disorganized), including by clinical and socioeconomic risk status. Gender differences were similar to “Western” findings. However, the growing evidence for convergent validity across cultural groups is tempered by low reporting of psychometrics. Narratives may sometimes reflect children’s unintended interpretations of the task and therefore not activate internal representations, or may reflect reality but lack equivalent meaning in coding schemes. We discuss how researchers and clinicians can enhance the validity of NSSTs by considering the role of culture in the sense-making process. Pending further validation work, NSSTs have the added potential to give a voice to young children from underrepresented backgrounds.

It’s a goddamn shame how evil the world is. And it’s incredibly sad that people become desensitized to it, but they do. Incremental improvement does very little for the present, but it’s the only feasible solution right now.
The alternative is to turn right wing and make everything exponentially worse. It’s incredibly sad that the people everywhere are powerless, but it’s a fact of life in the world right now.

Whichever ones are active at any local protests or actions. If none are, then a socialist organization that does not mince words about support for the resistance. If you are in college there are anti-imperialist orgs organized specifically by and for college students.
I would be more specific but it's more important to find an active org with a good line on Palestine (e.g. one that opposes Harris, supports fighting against Israel) than it is to join one specific national org.
